Registration
To register for the East Invite, please fill out a Registration form (Word OpenOffice html) and mail that, plus a check (made out to "East Ultimate,") to cover the Tournament Fee to the address below. The tournament fee includes field rental, food, water and other amenities.

The Tournament fee is $200 for an Open Team, $100 for a Women's team. If your team is from outside of Colorado, please send an e-mail to bill_burke73@yahoo.com for registration info.
The following UPA forms will be needed from each team on the day of the tournament.
All of these forms will be available as downloads from the UPA Event Forms Page.
- A filled out and up-to-date UPA roster -- every participant in the East
Invite should be on the roster. The roster that I need from your team is
the one under the heading
"Sanctioned Events" and is a "2007
Sanctioned Event Roster" (PDF)
(Excel).
On the roster, under "event," please write "East Invite."
On-Line Rostering will also be available. If your team will be using this to register, a paper roster are not needed. The below forms still will be.(If your kids are not planning on playing in States, other tournaments, or the juniors summer league, there is an option of paying a one-event UPA membership fee of 5 dollars, instead of a whole season's membership)
- A completed waiver for every participant.
- A completed chaperone waiver for each adult officially involved with the team (and each team should have at least one official chaperone at all times)
- A medical authorization form for everybody -- that's just so we don't have to put down any players for a sprained ankle, and can rather get them actual medical treatment.
