Registration
The tournament fee includes field rental, food, water and other amenities. Please send a check, made out to "East Ultimate," to our tournament director:

Please complete a registration form (Word OpenOffice html) and the following UPA forms from each team. If we have your completed forms in hand before Friday, April 1, 2005, your team will receive a $25 refund.
All of these forms will be available as downloads from the UPA Event Forms Page.
- A filled out and up-to-date UPA roster -- every participant in the East
Invite should be on the roster. The roster that I need from your team is
the one under the heading
"Sanctioned Events" and is a "2006
Sanctioned Event Roster" (PDF)
(Excel).
On the roster, under "event," please write "East Invite."
(If your kids are not planning on playing in States, other tournaments, or the juniors summer league, there is an option of paying a one-event UPA membership fee of 5 dollars, instead of a whole season's membership of 20 bucks.)
- A completed waiver for every participant.
- A completed chaperone waiver for each adult officially involved with the team (and each team should have at least one official chaperone at all times)
- A medical authorization form for everybody -- that's just so we don't have to put down any players for a sprained ankle, and can rather get them actual medical treatment.
